Canlyniadau Profion        Test Results

Yn gyffredinol, caiff canlyniadau unrhyw brofion gwaed, wrin neu belydr-x eu hanfon yn ôl at y practis o fewn diwrnod neu ddau i chi gael y prawf. Caiff y rhain eu trawsyrru i ni'n electronig o'r labordy i'ch ffeil bersonol a ddelir ar ein system gyfrifiadurol glinigol. Yna caiff y canlyniadau eu gwirio gan y meddygon a fydd yn penderfynu p'un a oes angen cymryd unrhyw gamau pellach. Gallwn roi'r canlyniadau i chi unwaith y byddant wedi cael eu prosesu sy'n hwyr y prynhawn fel arfer ar ôl 2pm. Byddwn yn rhoi gwybod naill ai "dim camau i'w cymryd" neu "canlyniad boddhaol" sy'n golygu bod popeth yn normal. Efallai y bydd y derbynnydd yn eich cynghori i "drefnu apwyntiad" ac os oes brys, caiff un ei drefnu tra byddwch ar y ffôn. Os oes sylw "siaradwch â'r meddyg neu'r nyrs", byddai hyn yn awgrymu bod angen i'r canlyniad gael ei drafod ond y byddai ymgynghoriad dros y ffôn yn ddigonol - bydd y derbynnydd yn nodi eich rhif ffôn a bydd y meddyg teulu neu'r nyrs yn eich ffonio'n ôl. Os bydd gennych apwyntiad dilynol, bydd y sylw ar y canlyniadau yn nodi "o dan ystyriaeth" ac yn yr achos hwnnw, caiff ei drafod yn ystod eich cyfarfod nesaf. Fodd bynnag, os byddwch yn poeni am y canlyniad neu'r sylw ar y prawf, bydd y derbynnydd yn ceisio eich rhoi trwodd i feddyg neu nyrs ar unwaith (os oes brys)  neu'n trefnu iddynt eich ffonio'n ôl pan fydd yn gyfleus. Rydym yn gwneud pob ymdrech i chi siarad â'r clinigwr a fu'n delio â chi'n wreiddiol fel bodd modd cysoni'r canfyddiadau clinigol yn fwy effeithlon.

In general the results of any blood, urine or x-rays will be sent back to the practice within a one to two days of your having the test. These are transmitted to us electronically from the laboratory to your personal file held on our clinical computer system. The results are then checked by the doctors who will decide if any further action is needed.

The results can be given to you once they have been processed which is usually later in the day after 2pm.  

You will either be told "no action necessary" or "result satisfactory" which means normal.  

You may be advised by the receptionist to 
"make an appointment" and if this is urgent one will be arranged for you whilst on the phone.  

If the comment is "speak to the doctor or nurse", this would imply the result needs to be discussed but that a phone consultation will suffice - the receptionist will take your phone number and the GP or nurse will ring you back.  

If you have an appointment for a follow up the comment on the results will be "being monitored" in which case they will be discussed at your next meeting.  However, if you are worried about the outcome or the comment of the test the receptionist will try to put you through to a doctor or nurse immediately (if urgent) or arrange for them to call you back when convenient.  We make every effort for you to speak to the clinician who dealt with you originally so that the result and the clinical findings can be synchronised more efficiently.
