New Patient Registration between 9am to 3pm only

Overseas patients - along with your other documentation,  only patients with a BRP card, or proof of BRP application will be allowed to register at the surgery.    If Coming into the UK on short term visa, the practice can see patients for urgent immediate care only but there will be a fee of £30 per consultation and £15 for an issue of private prescription.  

Uk citizens - please ensure you bring photographic ID, proof of address and your last current repeat slip( if applicable) 

Any patient wishing to register with the Practice may book a New Patient Health check with a Health Care Assistant. The health check is an opportunity for the patient and Practice to become acquainted. 

To register, a patient will be required to fill out a simple registration form giving details such as their name, address and their NHS number. To enable the medical staff to trace the patients previous medical records they will need to know the patient's previous Practice address. The GMS1 registration form and the patient Health questionnaire can be downloaded here and Health Questionnaire   Please note we cannot accept registrations without the completed Health Questionnaire.

The Practice Leaflet is available in English and Welsh and a copy can be collected from the reception desk. A larger print leaflet is available on request.

Please let the reception staff know if your details have changed (click on this link to open a Word document for completion) - for instance, new address or your mobile or home telephone number.   If you are happy to be contacted by email communication a valid email address can be added to your personal file. If you do not want us to text you with an appointment reminder please let us know.
